PAYMENT: Payment for services must be made in full upfront within 6 hours of the service unless discussed otherwise. Accepted payment methods include cash, debit, credit, invoice. Prices may include taxes. Failure to make timely payments may result in the discontinuation of service or rescheduling.
(1a) Lynn Mathers reserves the right to change the pricing and structure of sessions without the obligation to refund past, present, or future clients.

LATE AND EXTRA FEES: A $20 cancellation fee will apply if the Owner fails to give 24 hours' notice for any services unless otherwise stated. Late pick up or drop off will result in a 20$ fee, which may be waived for emergencies with documentation or by discussion with Lynn Mathers.

VACCINATIONS & HEALTH:
(3a) BOARDING/DAYCARE in THE DOG GIRL'S HOME: It is the Owner's responsibility to ensure all pets have current vaccinations or titers for Rabies, DHPP (Distemper, Hepatitis, Parainfluenza, Parvovirus). These vaccinations are not required for at home, however are recommended and The Dog Girl is not responsible for any illness or injury gained from lack of vaccination.
(3b) GENERAL SERVICES: Rabies vaccination is required. Other vaccinations are not required but recommended, Lynn Mathers is not responsible if your pet gets sick. I HEREBY AGREE TO INDEMNIFY LYNN MATHERS, THE DOG GIRL, AGAINST ANY CLAIMS MADE AGAINST LYNN MATHERS, THE DOG GIRL, AS A RESULT OF MY FAILURE TO INFORM LYNN MATHERS, THE DOG GIRL, OF ANY PRE - EXISTING MEDICAL CONDITIONS THAT MY PET MAY HAVE.

ON-TIME START: Lynn Mathers will strive to arrive on time. Delays due to previous appointments or unexpected traffic won't affect the duration of your bought time, but might impact the start time. Please consider this when scheduling your day.
(4a) CANCELLATIONS/RESCHEDULING: Lynn Mathers will strive to give clients as much notice as possible if canceling or rescheduling is required, and expects the same from clients.

VIDEO AND PHOTO RELEASE: Video or photos of owners' pets may be published on the website, social media or used in advertisement. By booking with Lynn Mathers, I acknowledge that I, my dog, and my family may be included in videos posted on the website or future training videos. All videos and photos are the property of Lynn Mathers. These video and photos may be used in any form, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional materials. Lynn Mathers will keep secure and confidential any keys, pass codes, access numbers and personal information. If there is a safety reason or personal request to not release video and photo please contact Lynn Mathers and make aware of this BEFORE booking in.
(5a) Phone Usage: Lynn Mathers, The Dog Girl, may be using her phone while providing service, the owner accepts and understands this and no complaints, reports, or otherwise defamation will be made against Lynn Mathers, for phone usage, unless made with proof of gross negligence or abuse for pets under The Dog Girl, Lynn Mathers care.
(5b) Video Cameras etc: It is under the owners responsibility to disclose any cameras on the property, including within the home, where The Dog Girl will be providing services. Cameras are permitted, so long as they have been disclosed. Cameras in private areas such as bathroom and bedroom are not permitted and will immediately be considered a breach of contract. Cameras that are in bedrooms where the pet stays may be permitted but will need to be turned off during overnight stays or otherwise covered.
The Owner, does NOT have permissions to share, post, or otherwise use this footage unless in proof of gross negligence, abuse, or emergency for pets under The Dog Girl, Lynn Mathers care. Failure to comply with this agreement will be considered a breach of contract resulting in the termination of this contract and/or costs and damages payable by me, OWNER, to Lynn Mathers with no refunds.

REFUNDS: Refunds may be discussed and given at Lynn Mathers discretion. This is typically half money back based upon but not limited to the situation.

PACKAGES: Lynn Mathers' dog services packages have a six-month validity. This means that if you do not respond for 6 months or more, your package will be invalid and no refund will be given. Keep in communication if there is any reason you will not be able to use said package including emergencies. This may be waived with permission.

WEATHER: Lynn Mathers will contact clients to reschedule their session if the weather is deemed too dangerous.

PRIVACY POLICY: ALL PERSONAL CONTACT INFORMATION SUCH AS ADDRESS, EMAIL, PHONE NUMBER OF THE CLIENT WILL NOT BE SHARED WITH ANYONE OUTSIDE OF THE DOG GIRL AND HER BUSINESS.

DUTY TO DISCLOSE: I, the OWNER of dog in service, represent that I have disclosed and shall continue to disclose, any and all medical conditions or any other conditions, including, but not limited to, personality concerns or behaviors that may affect, limit, or prevent my pet’s ability to participate in services provided by Lynn Mathers. I understand that Lynn Mathers is relying on and will rely on those representations to provide a safe environment for both humans and animals.

EMERGENCY VET CARE:  In the event of an emergency, where the owner cannot respond, or does not respond fast enough to address the emergency at hand, I, the OWNER of said dog or pet in service, agree to allow Lynn Mathers to bring my pet to a veterinarian. Medical treatment may require transportation of my pet to receive care and I hereby authorize such transportation. Every effort will be made to contact the OWNER. I hereby agree that I am fully responsible for the cost of any medical treatment and transportation required to save my pet. I agree that I am aware of all risk and consequences associated with the services provided by Lynn Mathers. Furthermore, I agree that I am aware of all risk and consequences associated with any decisions made by Lynn Mathers, relating to the medical care and transportation of my pet.

WAIVER OF RELEASE AND INDEMNIFICATION. I, OWNER, RELEASE, WAIVE, DISCHARGE, INDEMNIFY AND AGREE TO HOLD LYNN MATHERS HARMLESS FOR ANY AND ALL MANNER OF DAMAGES, INJURY, CLAIMS, LOSS, LIABILITIES, COSTS OR EXPENSES, LEGAL FEES, COURT FEES, CAUSES OF ACTION OR SUIT, WHATSOEVER IN LAW OR EQUITY, ARISING OUT OF OR RELATED TO THE SERVICES PROVIDED BY LYNN MATHERS INCLUDING WITHOUT LIMITATION SUCH AS: ANY INACCURACIES IN ANY STATEMENTS MADE BY MYSELF OR INFORMATION PROVIDED BY ME TO LYNN MATHERS, ACTIONS OF MY PET, INCLUDING BUT NOT LIMITED TO DESTRUCTION OF PROPERTY, DOG BITES, INJURY, AND TRANSMISSION OF DISEASE, AND ANY ACTION BY MYSELF THAT IS IN BREACH OF THE TERMS OF THIS AGREEMENT.

Affirmation: Each time I, the OWNER, request a service by Lynn Mathers I am re-affirming the terms of this agreement, and the truthfulness and accuracy of all statements I have made in this agreement. I HAVE READ AND FULLY UNDERSTAND THE TERMS OF THIS AGREEMENT AND UNDERSTAND THAT I WILL GIVE UP SUBSTANITAL RIGHTS BY SIGNING IT. I HAVE SIGNED THIS AGREEMENT FREELY AND VOLUNTERILY WITHOUT ANY INDUCEMENT, ASSURANCE OR GURANTEE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LILABILITY LYNN MATHERS. TO THE GREATEST EXTENT PERMITTED BY LAW. I FURTHER AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID OR UNENFORCEABLE, THE REMAINDER OF THIS AGREEMENT SHALL REMAIN IN FULL FORCE AND EFFECT.

Sole Agreement: This writing represents the sole agreement between Lynn Mathers and the Owner

Booking Agreement: By booking in with Lynn Mathers for any pet services, you acknowledge, understand, and agree to follow these terms.

ENTIRE AGREEMENT. Any violation of the aforementioned terms will be considered a breach of contract resulting in the termination of this contract and/or costs and damages payable by me, OWNER, to Lynn Mathers. The agreement is listed above in its entirety. There are no oral representations made outside of this contract. No oral agreements can be made to vary this contract. Any amendments must be in writing and signed by Lynn Mathers and I, the Owner.
